Entering Personal Unlock Key (PUK) on iPhone

1. Device will advise Type PUK to Unlock SIM Card (This is due to entering the incorrect
SIM PIN number in excess of 3 times)
2. Enter PUK Code  and tap OK  (You can find the PUK number either on the original SIM
packaging or on the Optus Help Website)
3. Enter New SIM PIN and tap OK
4. Re-Enter New SIM PIN and tap OK  (This is to confirm you entered the PIN you desired
correctly)

Note: You can identify this by the iPhone displaying Locked SIM in the top left hand corner;
if your iPhone is not displaying the unlock message go to the Main Menu and tap Phone, dial
any number and you will be prompted to enter the PUK code.
